Mons pubis liposuction is a liposuction procedure that aims to reduce excess fat in the upper pubic area above the labia majora. It is sometimes confused with monsplasty, which is similar but not quite the same. Mons liposuction removes fat only. On the other hand, monsplasty frequently involves liposuction of fatty tissue located on the mons pubis and reduces excess skin for recontouring of the general area. This procedure involves a longer scar and is reserved for patients with laxity.
Many patients choose the mons liposuction procedure when excess fat in the mons pubis area affects their comfort, clothing options, and confidence. In some cases, the excess tissue and “puffiness” in this area is most visible and uncomfortable in tight clothing— jeans, form-fitting dresses or skirts, yoga pants and exercise tights.
Mons liposuction is frequently combined with other treatments, such as tummy tuck surgery or liposuction on other areas of the body. Many patients are mothers who are done having children or those who’ve experienced a significant weight loss. They frequently report this area as disproportionate, even when they are at a lower weight or since childhood. Everyone stores fat differently, so sometimes excess fat in this area is simply a matter of one’s natural anatomy as well.
Mons pubis liposuction can be an excellent solution for individuals who feel self-conscious about excess fat in the pubic area above their labia. This procedure, which is commonly performed by board certified plastic surgeon Dr. Cynthia Diehl, can give you back your confidence and help you feel more comfortable in your favorite clothes.

While mons pubis liposuction is more common among women, men can also benefit from this procedure. The treatment helps men address excess fat in the pubic area, which may interfere with wearing certain clothing or athletic activities. Men who undergo this procedure typically report improved comfort and a more streamlined appearance in the lower abdominal area.
Dr. Diehl will begin the mons lipo procedure by administering local anesthesia to make two small (3-4mm) incisions. Although not commonly necessary, Pro-Nox (nitrous oxide/oxygen mixture also known as “laughing gas”) is also available to improve their comfort and reduce anxiety.
Once Dr. Diehl has made the small incisions the area is filled with tumescent fluid to numb and “tumesce” the entire mons. This is always the first step of any liposuction procedure and patients describe it as a strange feeling. The magic of this step is that it has numbing medicine mixed in with it. Once this step is done the patient will only feel pressure sensations and no discomfort. The small liposuction cannulas will be used to remove excess fat and create a smoother profile and more contoured pubic region. Finally, the incisions will be closed with dissolvable stitches and the patient will be placed in a compression garment. The patient is monitored for vital signs and comfort throughout the procedure.
Dr. Diehl is driven by the desire to better the lives of her patients. Dr. Diehl is one of the few female and Spanish-speaking plastic surgeons in Raleigh.
She is board certified by the American Board of Plastic Surgery and was previously certified by the American Board of Surgery. Dr. Diehl is trained in cosmetic facial, breast, and body procedures, including SmartLipo MPX, an FDA approved laser liposuction technology.
She also provides non-surgical cosmetic treatments including CoolSculpting®, Thermi®, miraDry®, and injectables to patients in Raleigh and the surrounding area.
Candidates for mons pubis liposuction generally have excess fat in the pubic area—specifically, on the pubic mound or “mons pubis,” which is the fatty pad covering the pubic bone. For some people this area does not respond to diet and exercise well. The mons lipo surgical procedure may be ideal for individuals who experience self-consciousness or feel that this area is disproportionate and limiting their clothing options.
Mons liposuction can be suitable for both men and women, although it is most commonly performed on women. Dr. Diehl will assess each patient’s individual anatomy and aesthetic goals to determine if mons pubis liposuction is the best option for their needs.
Recovery from mons pubis liposuction varies by patient but is typically brief. Most patients are able to resume normal activity whenever they are comfortable. The most important instruction is to maintain some sort of compression to the area to help resolve the swelling from the fluid we put in to do the procedure and the patient’s natural swelling response to the surgery. Mild bruising is not uncommon and should resolve for most within one to two weeks.
Dr. Diehl will provide detailed aftercare instructions, which should be closely followed. You will need to wear a compression garment to reduce swelling and support healing. This should typically be worn for three to four weeks. Most patients start to see changes and improvement to the area within a few weeks, with the final results several months later with resolution of the swelling and the tightening that happens over time.
The mons pubis liposuction price varies depending on the extent of fat removal needed and any additional procedures that are simultaneously performed. At your consultation, Dr. Diehl will discuss the details of your surgery and provide a more precise estimate of cost based on your individual needs and goals.

While mons pubis liposuction focuses on removing excess fat, if you would like to remove excess skin from this area as well, that is a slightly different plastic surgery procedure called monsplasty. Typically patients (even patients with laxity) shrink the skin proportionally to the new contours with liposuction alone. If someone wants the area “tightened” then skin excision may be needed.
Often, monsplasty involves mons pubis liposuction, but reduction of excess skin in the pubic area is sometimes needed as well—for example, if a patient has lost a significant amount of weight, has had one or more pregnancies with lots of stretch marks and looseness, or who genetically has significant laxity.
Discussing your goals with Dr. Diehl at a consultation appointment can help determine if liposuction alone will achieve the contour you desire or if a pubic lift might be necessary.
After mons pubis liposuction, most patients start noticing initial improvements within a few weeks. However, it will take longer (sometimes several months) to see your final results. This is usually due to residual swelling and the shrinking of the tissue that is slower.
The timeline for recovery and final results will vary based on individual healing rates as well. It also matters how closely post-operative care instructions are followed. Dr. Diehl will provide guidance on managing swelling and achieving optimal results after surgery. Follow these guidelines closely for the best outcome.
Excess fatty tissue in the mons pubis area can be uncomfortable—both emotionally and physically. If this is something you struggle with, mons pubis liposuction may be able to help.
